ABSTRACT

To scientifically evaluate the intervention effect of Chinese medicine preventive administration(combined use of Huo-xiang Zhengqi Oral Liquid and Jinhao Jiere Granules) on community population in the case of coronavirus disease 2019(COVID-19), a large cohort, prospective, randomized, and parallel-controlled clinical study was conducted. Total 22 065 subjects were included and randomly divided into 2 groups. The non-intervention group was given health guidance only, while the traditional Chinese medicine(TCM) intervention group was given two coordinated TCM in addition to health guidance. The medical instructions were as follows. Huoxiang Zhengqi Oral Liquid: oral before meals, 10 mL/time, 2 times/day, a course of 5 days. Jinhao Jiere Granules: dissolve in boiling water and take after meals, 8 g/time, 2 times/day, a course of 5 days, followed up for 14 days, respectively. The study found that with the intake of medication, the incidence rate of TCM intervention group was basically maintained at a low and continuous stable level(0.01%-0.02%), while the non-intervention group showed an overall trend of continuous growth(0.02%-0.18%) from 3 to 14 days. No suspected or confirmed COVID-19 case occurred in either group. There were 2 cases of colds in the TCM intervention group and 26 cases in the non-intervention group. The incidence of colds in the TCM intervention group was significantly lower(P<0.05) than that in the non-intervention group. In the population of 16-60 years old, the incidence rate of non-intervention and intervention groups were 0.01% and 0.25%, respectively. The difference of colds incidence between the two groups was statistically significant(P<0.05). In the population older than 60 years old, they were 0.04% and 0.21%, respectively. The incidence of colds in the non-intervention group was higher than that in the intervention group, but not reaching statistical difference. The protection rate of TCM for the whole population was 91.8%, especially for the population of age 16-60(95.0%). It was suggested that TCM intervention(combined use of Huoxiang Zhengqi Oral Liquid and Jinhao Jiere Granules) could effectively protect community residents against respiratory diseases, such as colds, which was worthy of promotion in the community. In addition, in terms of safety, the incidence of adverse events and adverse reactions in the TCM intervention group was relatively low, which was basically consistent with the drug instructions.

ABSTRACT

Objective To observe the clinical efficacy of acupuncture at three tongue points plus Vitalstim electrical stimulation in treating deglutition disorder after cerebral stroke. Method Seventy patients with deglutition disorder after cerebral stroke were randomized to treatment and control groups, with 35 cases in each group. Both groups received conventional medication treatment. In addition, the treatment group was intervened by acupuncture at three tongue points plus Vitalstim electrical stimulation, while the control group was intervened by swallowing rehabilitation training. Kubota water swallowing test assessment rating, Standardized Swallowing Assessment (SSA) score and Swallow Quality-of-life Questionnaire (SWAL-QOL) score in the two groups were observed before and at the end of the treatment, and the clinical efficacies of the two groups were compared. Result The Kubota water swallowing test assessment rating, SSA score and SWAL-QOL score all showed a significant change after the treatment in the two groups (P<0.05). After the treatment, the Kubota water swallowing test assessment rating, SSA score and SWAL-QOL score in the treatment group were significantly different from those in the control group (P<0.05). The total efficacy rate was 87.5% in the treatment group and 73.3% in the control group, and there was a statistically significant difference between the two groups (P < 0.05). Conclusion Acupuncture at three tongue points plus Vitalstim electrical stimulation is an effective approach in treating deglutition disorder after cerebral stroke. It can improve swallowing function and quality of life in the patients.

ABSTRACT

Objective: To investigate the mechanism of fructose-induced monocyte chemoattratant protein-1(MCP-1) production in HK-2 cells. Methods: The HK-2 cells were divided into fructose incubated (1, 5 and 10 mmol/L) group, fructose and ketohexo-kinase inhibitor (KHK-IN) coincubation (fructose 5 mmol/L, KHK-IN was 12, 100 and 1000 nmol/L, respectively) group, uric acid incubation (5, 15 and 50 mg/dL) group, fructose and allopurinol co-incubation (fructose 5 mmol/L, allopurinol were 0.01, 0.1 and 0.5 mmol/L) group, uric acid and allopurinol co-incubation (uric acid 50 mg/dL, allopurinol respectively 0.01, 0.1and 0.5 mmol/L) group, H2O2 incubation (0.1 and 0.3 mmol/L) group, fructose and N-acetylcysteine (NAC) coincubation (fructose 5 mmol/L, NAC respectively 5, 10 and 50 mmol/L) group, and uric acid and NAC co-incubation (uric acid 50 mg/dL, NAC was 5, 10 and 50 mmol/L, respectively) group. The quantitative PCR method and Western blotting method were used to observe the expression of MCP-1 mRNA and protein. The effects of fructose and uric acid on the production of ROS in HK-2 cells were observed by using a fluorescent probe. Results: Fructose doseand time-dependently induced MCP-1 gene transcription and protein production in HK-2 cells, which could be blocked by the ketohexo-kinase blockers. Exogenous uric acid induced MCP-1 production in HK-2 cells. Allopurinol inhibited fructose, but not exogenous uric acid-induced MCP-1 expression. Both fructose and uric acid induced ROS generation. Incubation with H2O2 promoted MCP-1 production in HK-2 cells. NAC completely inhibited MCP-1 production induced by fructose and H2O2. Conclusion: Catalyzed by the ketohexo-kinase, fructose resultes the production of MCP-1 through uric acid and reactive oxygen species.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the value of blood lactic acid (BLA) as a predictor for the severity and prognosis of neonatal shock.</p><p><b>METHODS</b>A total of 326 neonates with shock were enrolled and divided into three groups based on the severity, namely mild group (n=147), moderate group (n=105), and severe group (n=74). BLA level was measured during and early after (about 6 hours later) fluid resuscitation, and lactate clearance rate (LCR) was calculated. The receiver operating characteristic (ROC) curve was applied to evaluate the predictive value of BLA in neonatal shock.</p><p><b>RESULTS</b>BLA level was high in all subjects prior to treatment, and was highest in the severe group and lowest in the mild group (P<0.01). BLA level was significantly higher among patients with septic shock than among those with hypovolemic, cardiogenic, and asphyxiating shock (P<0.05). BLA level was significantly reduced in patients in recovery after treatment (P<0.05). Mortality was significantly lower in patients with BLA level ≤4 mmol/L or LCR ≥10% than in those with BLA level >4 mmol/L or LCR <10% (P<0.01). BLA at 11.15 mmol/L had 100% sensitivity and 96.8% specificity in predicting severe shock. BLA at 10.65 mmol/L had 88.9% sensitivity and 74.1% specificity in predicting the prognosis (survival or dead) of newborns with shock.</p><p><b>CONCLUSIONS</b>In neonates with shock, arterial BLA level increases as the disease severity increases and is associated with prognosis, so it is a useful predictor of the severity and prognosis of neonatal shock.</p>

ABSTRACT

<p><b>BACKGROUND</b>Rathke's cleft cyst (RCC) is one of the most common incidentally discovered sellar lesions, while symptomatic cases are relatively rare. Surgical treatment is recommended for symptomatic patients to drain the cyst content and to remove the capsule safely. The aim of this study was to clarify the clinical features, surgery considerations and therapy outcomes of symptomatic RCCs.</p><p><b>METHODS</b>Totally 42 patients (19 males and 23 females) were retrospectively reviewed with the diagnosis of RCCs under surgery resection at the Affiliated Hospital of Medical College, Qingdao University between January 2005 and December 2010.</p><p><b>RESULTS</b>Patients' age ranged from 6 to 67 years (mean of 41.6 years). The duration of symptoms ranged from 4 days to 10 years. Headache (69%), visual impairment (36%), and pituitary dysfunction (10%) were the most common presenting symptoms. The maximum diameter of cysts ranged from 6.0 to 46.7 mm (mean of 20.07 mm). Of the 42 patients, 36 underwent endonasal transsphenoidal approach and the others underwent transcranial approach. Thirty patients had a subtotal resection and decompression, while 12 patients had a total cyst resection. Cysts of 28 patients were lined by simple cubical or columnar epithelium, and cysts of 34 patients were filled by amorphous colloid material, that was the characteristic of RCCs. The majority of patients presented with a simple headache, and 93% of this group experienced a complete improvement after surgery. Twelve of 15 patients (80%) with preoperative visual deficits experienced an improvement in their vision after surgery. All of those patients with pituitary dysfunction experienced an improved endocrine status. The endocrinological complication usually was diabetes insipidus, and postoperative transient diabetes insipidus occurred in 13 (31%) patients without any permanent diabetes insipidus. The overall recurrence rate was 7% at a mean follow-up of 22 months (range 12 - 60 months).</p><p><b>CONCLUSIONS</b>Surgical treatment is to drain the contents of the cyst and to remove the capsule as much as possible under the precondition that does not increase the complications. Biopsy and decompression procedures are recommended for most cases.</p>

ABSTRACT

Epidermal growth factor receptor (EGFR) is a classic protein tyrosine kinase receptor, which plays an important role in cell proliferation, survival, adhesion, differentiation and apoptosis. Abnormality of EGFR and its signaling are closely associated with tumor initiation and development. Many environmental physical and chemical agents can interfere with EGFR and its signal transduction pathways via affecting its phosphorylation, conformation and function, or distribution on cell membrane, finally influencing gene expression and cell fate. This review focuses on the recent progress of above aspects for further understanding of epigenetic mechanisms of cellular stress and carcinogenesis related with environmental agents.
